   • SSC-PADI: Print, fill in with handwriting, sign and get your physician to sign PDF
     or
   • SSC-PADI-form: Fill in most fields by typing, print, sign and get your physician to sign PDF

Note:  PADI Certification is not guaranteed and requires completing all necessary material with proficiency.

$2000 per 2-week session, with an option for extensions.
